To begin let me greet all of you and welcome you to this presentation on the basic tenets of care coordination. There is coordination of care which is a crucial task that entails the management of patients’ services and information by all stakeholders involved in the care of a particular patient to enhance the safety of the patient and improve the outcomes of care. This paper seeks to expound on the effects of care coordination on the patients and overall quality of health services to provide better revelations on these principles. White et al, (2021) posited that proper care coordination helps in a decrease of hospitalization by 25/60, and also enhances satisfaction of patients by 30/60. During this presentation, some of the most important topics related to care coordination and the patient’s experience are the methods of collaboration with patients & families, change management factors affecting patient experience, ethical considerations in creating care coordination, and the impact of healthcare policies on patient experience.

As the subject of my presentation, I will be focusing on the functions of the nurse in the coordination of care, the partnership between the nurses and the patients and the families, health politics and their consequences, and the principles of ethical practice in patient/accountable center-oriented care.
